Diabetes is one of the top 10 leading causes of death in the United States (National Center for Health Statistics, 2016). This multi-factorial chronic disease requires continuous glucose monitoring, behavior modification, and daily self-management. However, due to the complicated illness management and a lack of preventative approach, its morbidities and mortalities continues to rise (Powers et al., 2015). In the U.S. alone, there are 29 million people diagnosed with diabetes and nearly 86 million living with a pre-diabetic condition (Kirtland, Cho, & Geiss., 2015). The medical costs for diabetes management also heavily burdens the US federal budget. The estimated cost of diabetes in 2012 was $245 billion, including diagnoses, medical cost, management, and prevention (American Diabetes Association, 2013). Therefore, there is a need for the implementation of comprehensive diabetes self-management education and support for diabetic patients.

One of the Healthy People 2020’s leading health indicators is the clinical preventive services by achieving the reduction of Hemoglobin A1c (Hgb A1c) greater than 9 percent among adults 18 and over with diabetes (Healthy People 2020, 2017). In order to address this issue, the Polk Dalton clinic has implemented the Point-of-Care education project. The project includes, assessing barriers to diabetic management among patients with diabetes and assisting them to develop individualized care through promoting reactive interaction between both healthcare providers and the patients. These elements allow healthcare providers develop a daily healthcare plan to achieve long term self-care management goals.
